Scheduling Appointments
Appointments can be made online.

We ask that all patients arrive 10-15 minutes before their scheduled appointment time in order to complete any necessary paperwork.

We have a 15-minute grace period for all appointments. If you are late you may be asked to reschedule to a later time or a later date so that the patients who are on time are not inconvenienced.

Cancellation & No-Show Policy
We schedule our appointments so that each patient receives the right amount of time to be seen by our physicians and staff. That’s why it is very important that you keep your scheduled appointment with us, and arrive on time.

As a courtesy to our office as well as to those patients who are waiting to schedule with the physician, should you need to cancel or reschedule your appointment, please give us at least 24 hours notice.
Failure to arrive for or cancel / reschedule your appointment with 24 hour advance notice will result in a $50 "No-Show" charge to your account. This “No-Show" charge is not reimbursable by your insurance company and you will be billed directly for it. This broken appointment fee will need to be paid on or before your next office visit.
Co-Pays & Insurance
We accept most local and many national insurance plans.

Co-pays must be paid at the time of service, even with secondary policies. New patients must have co-pays on date of service. We accept cash, and most major credit cards and debit cards. ​​We DO NOT accept personal checks.

​We ask that you provide complete and accurate information about your insurance so that we may submit claims to your insurance carrier and receive payments for services. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance or other deductible amounts.

Prescriptions
Prescriptions are E-faxed to the pharmacy after your visit and it is your responsibility to pick it up at pharmacy.

Contact your pharmacy to inquire if it is ready as some prescriptions may take 4-5 hours to fill.

If you have a Mail Order prescription program through your insurance, we will mail or fax the prescription upon your request.
After Hours & Emergency Care
If you are experiencing an emergency, dial 911 or head to the nearest emergency room.
If you are asked to wait for the doctor, please try to understand that as a specialist, Dr. Brooks may be confronted with emergency cases which could delay the schedule. If this happens we always try to contact our patients to come in at a later time or give the option to reschedule at their convenience.
As your insurance may have limits on the doctors you can see or what services you can receive, It is your responsibility to know what your insurance provides. Please read your policy carefully and know its provisions, as well as its exclusions. The doctor and staff cannot be knowledgeable about the coverage of the hundreds of health care plans presented.​​
